• Confidentiality

    Any information that you reveal to a therapist is considered "privileged communication" and that information is kept confidental. Mental health professionals are not allowed to release any information about clients unless a “Release of Information” form is signed, which permits the transfer of specific information to a specified individual. The form is valid for 90 days.

    There are certain situations in which information about clients may be released with or without their permission. These situations are as follows:

    1. When children are neglected, physically abused or sexually abused, the proper authorities (e.g. police, State Division of Family Services) must be notified.
    2. In emergency situations, such as a threat of homicide or suicide, in which there is potential danger to the client or other individuals confidentiality may be broken.
  • Insurance

    Cynthia Travnicek accepts nearly all major insurance plans. Cynthia is a preferred or contracted provider for most major insurance companies.

    Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

    • Do I have mental health insurance benefits?
    • What is my deductible and has it been met?
    • How many sessions per year does my health insurance cover?
    • What is the coverage amount per therapy session?
    • Is approval required from my primary care physician?

    As a contracted provider, I agree to a discounted rate from your insurance company. I will submit your claims for you through a private billing service. You are responsible for your deductible, coinsurance, or copay amounts at the time of service.  We accept cash, check, credit card or debit card.

    What if I don't have insurance or you don't accept my insurance?   We accept Self-Pay clients. 

  • Cancellations and Missed Appointments

    We will Text or Email an appointment reminder 48 hours prior to your visit. All cancellations or rescheduling requests must be received by our office 24 hours prior to your scheduled appointment. Payment of $60.00 will be charged for all missed appointments and cancellations received less than 24 hours from your scheduled appointment date and time. Please note that charges for missed appointments cannot be billed to your insurance company, and will be your financial responsibility.
